PATIENT INFORMATION

You know you need physical therapy, but now what? In Texas, we are able to evaluate and treat patients without a referral or prescription up to 15 days. If you have experienced a recent injury, been diagnosed with a new condition, or have a chronic pain that has limited your daily function, give us a call today at 409-237-4181 to be seen as soon as possible by one of our dedicated physical therapists. If more visits are needed, our therapists will guide you through the process to make sure you receive the care you need.

If you know that you will be needing therapy in the near future, for example you have an upcoming orthopedic surgery, your doctor can send the referral straight to our clinic. But it isn't necessary for the referral or prescription to be sent from your doctor to us. If you have been given a referral for physical therapy, call to schedule an appointment and bring the referral with you on your first visit.

Finally, many doctor's offices have clinics they work closely with. If your doctor recommends a different clinic but you prefer to be treated at Proficient Rehab, let your doctor know you would like a paper copy of the referral. It is your choice who provides your care, whether that is here at Proficient Rehab or any other clinic in our community. If we do not currently work with your physician, we would love to change that!
